Agent staleness issue: Both tinker-worker (ID: 4) and tinker-reviewer (ID: 5) show stale availability_updated_at timestamps and do not respond to send_message_to_agent despite success returns. Reviewer has 7 pending_audit tickets but remains idle. Issue documented in escalation ticket #119. Cannot be fixed via orchestrator workflow - requires agent process restart or message infrastructure fix.
