## Context
Orchestrator is assigning tickets with already-merged PRs to agents. The orchestrator workflow skill explicitly states:
- **Priority 0:** Check pending_audit/pending_approval tickets → IF MERGED, move to done
- **Scenario B:** Before assigning reviews → VERIFY ticket is NOT merged

The orchestrator has `gh` CLI available and can check PR status, but is not performing this check.

## What You're Building
Fix orchestrator behavior so it checks PR merge status before assigning tickets. Tickets with merged PRs should be transitioned to `done`, not assigned.

## Acceptance Criteria
- [ ] Orchestrator checks PR status before assigning tickets
- [ ] Merged tickets are moved to `done`, not assigned to agents
- [ ] Check happens for both pending_audit and pending_approval tickets

## Technical Notes
- Orchestrator has GH_TOKEN and can use `gh pr view <url> --json merged,mergedAt,state`
- Workflow skill at `.claude/skills/orchestrator-workflow/SKILL.md` already documents this behavior
- Tickets have `pull_request_url` field
- This may require updating the orchestrator's skill/banner to make the check more prominent
