## Problem

11 tickets are stuck in `pending_approval` status with PRs. They have `approve`/`reject` transitions (not `pass_audit`/`fail_audit`), which suggests a different approval workflow.

## Impact

- Worker agent cannot start work on Epic #132 (Researcher agent) because ticket #133 is in pending_approval
- Ticket #133 blocks subtasks #134-#138
- No work available for idle worker
